JOB DESCRIPTION

LPN

The Household Nurse delivers, coordinates, evaluates and is accountable for the total nursing services for household residents during assined shifts.  The Household Nurse performs nursing functions that require medication administration, observing residents' condition and communicating them to the clinical supervisor or physician, and treatments; and also takes part in Household activities with and for residents.  Work is performed according to the community's policies and procedures within the scope of practice o the Virginia Board of Nursing guidelines and in a safe and efficient manner.
RESPONSIBILITIES

  • Responsible for the total nursing care of residents assigned during the tour of duty
  • Receives report from off-going shift, gives report to oncoming shift, and makes rounds
  • Receives physician’s orders and carries out orders according to established policy and procedures
  • Stays knowledgeable of resident conditions and reports significant change to Nurse Manager or Nursing Supervisor and/or Physician
  • If assigned to the HCU, responds to emergency calls of apartment residents in the absence of the Nurse Manager
  • Administers medications following all regulations and established policy and procedures
  • Orders and stores medications according to the established pharmaceutical policies and procedures
  • Contacts Nurse Manager or DON and/or Physician if there are any questions regarding resident medication or treatment, implements established stop-order policy, and removes all outdated and discontinued drug medication bins and places in designated area
  • Maintains and revises, as needed, nursing care plan of residents assigned
  • Maintains current scheduled and controlled drug record following all federal and/or state laws
  • Administers treatment according to Physicians’ orders
  • Maintains resident clinical records following established policies and procedures
  • Supervises and evaluates assigned geriatric Certified Nursing Assistants
  • Ensures that all equipment is working properly and that a safe environment is maintained in resident areas and promptly completes maintenance request forms as needed
  • Records required information in “Report Book” and Nurses’ Notes, records monthly summaries on resident charts as assigned, and records weight and blood pressure monthly as assigned
  • Assesses resident injuries and renders first aid, makes decisions for appropriate referral to doctor and/or ER, and completes Incident Report
  • Makes shift rounds with physicians
  • Maintains a physical, social, and psychological environment conducive to the well being of the resident
  • Keeps Dining Services informed of diet changes
  • Regularly checks residents receiving IV fluids
  • Supports and participates in in-service education and attends staff meetings regularly
  • Keeps Nurse Manager and DON informed of major problem areas and recurrent grievances
  • Bathes and feeds acutely ill residents and/or helps geriatric nursing assistants with resident care as necessary
  • Administers subcutaneous fluids as needed (RN to administer intravenous fluids unless LPN has had specialized training)
  • Makes resident assignments to geriatric nursing assistants such as cleaning of baths, utility rooms, Nurse's stations, and linen rooms
  • Encourages motivation, socialization, and rehabilitation of residents and helps create a “home” atmosphere
  • Implements established resident care policies and procedures
  • Performs other duties as assigned
QUALIFICATIONS

  • Licensed as a Licensed Practical Nurse (LPN) or Reginstered Nurse (RN) in the state of Virginia
  • Communication and interpersonal skills to develop relationships with residents, staff, and providers
  • Genuine desire to work with the elderly
  • An up-beat and engaging personality
  • Must possess a spirit of cooperation and enthusiasm and be willing to perform non-professional duties
